What a good intake form should include
The job of an intake form is simple: help you understand who your client is, what they want, and what might get in the way — before you meet. The best forms are short, specific and easy to answer honestly.
- Contact basics — name, email, phone, and how they prefer to be contacted.
- Their reason for coming — in their own words, not a checkbox.
- Top 3 goals for the next 3 months — specific enough to measure later.
- A lifestyle snapshot — sleep, water, movement, stress and energy, rated simply.
- Current routines — meals, work schedule and anything that shapes their week.
- Health notes and preferences — anything you should know, phrased openly.
- A clear consent line — that coaching is not medical advice, diagnosis or treatment.

A sample structure that works
Keep it to one or two pages. Start with the easy factual questions, move to goals and lifestyle, and end with the open questions and consent. Clients finish short forms; they abandon long ones. The downloadable template above follows exactly this order.
How to use it in your practice
Send it 2–3 days before the first session so you have time to read it properly. Refer back to their goals at every check-in — it keeps sessions focused and shows clients you listened. When you're ready, the same form (with your logo and colours) is inside Wellness Vault Pro, alongside goal worksheets, session notes and progress trackers.

Common questions
- When should a client fill in the intake form?
- Before your first session, ideally 2–3 days ahead. That gives you time to read it properly and shape the first conversation around what they wrote, instead of spending the session collecting basics.
- How long should an intake form be?
- One to two pages. Enough to understand goals, lifestyle and anything you should know — short enough that clients actually finish it. You can always go deeper in later sessions.
- Can I add my own branding to this template?
- Yes. Inside Wellness Vault Pro you can add your logo, colours and business details to the intake form and every other client form, then download a branded PDF.
